Accept URL parameters​

Accept URL parameters to set field values. Use field Code value as parameter name.

Survey mode​

Survey mode allows filling up the form only one time.

Redirect URL​

Redirect to specified url after successful submission.

Result Approval Settings​

Enable result approval controls​

You can switch submission result status: pending, approved, completed or not approved.

Enable approval status notification​

Send customer notification email on submission result status change.

Bcc e-mail address​

Send blind carbon copy of notification to specified address. You can set multiple addresses comma-separated.

Result NOT approved notification email template​

Set custom e-mail template for NOT approved status notification.

Result approved notification email template​

Set custom e-mail template for approved status notification.

Result completed notification email template​

Set custom e-mail template for completed status notification.

reCAPTCHA Settings​

Captcha mode​

Configures how the captcha is being displayed:

  • Default. The value is used from the system configuration.

  • Auto. The captcha is displayed for non-logged in customers only.

  • Always on. The captcha is always displayed.

  • Off. The captcha is disabled for the form.
